چکیده

By 2000, more than 1 202 000 percutaneous and 519 000 surgical revascularization procedures were being performed annually in the United States.1 The 3 potential reasons to recommend myocardial revascularization are (1) to alleviate symptoms of myocardial ischemia, (2) to reduce the risks of future mortality, and (3) to treat or prevent morbidities such as myocardial infarction, arrhythmias, or heart failure. To minimize biases, this article focuses on data from prospective, randomized, controlled trials (RCTs) comparing coronary artery bypass graft (CABG) surgery, percutaneous transluminal coronary angioplasty (PTCA), stents, and medical therapy for chronic coronary artery disease (CAD). The data are interpreted in the context of a conceptual framework based on patient risk, methodological characteristics of the evidence, and the occurrence of clinically relevant end points.

Economic considerations in managing patients with chronic stable angina.

OBJECTIVE To quantify the economic burden of chronic stable angina in the United States, characterize recent trends in the use of coronary revascularization, and compare the clinical outcomes and long-term costs of percutaneous coronary intervention (PCI), coronary artery bypass grafting (CABG), and medical management in patients with stable angina. SUMMARY The direct and indirect costs of st...

Comparison between Revascularization and Optimal Medical Therapy in Patients with Stable Angina Pectoris

Background and Purpose: Regarding usefulness of revascularization versus optimal medical therapy in patients with stable angina pectoris, data are challenging. The aim of this 12-month follow-up study was to compare the survival benefit associated with revascularization versus optimal medical therapy on the patients with stable angina pectoris. Materials and Methods: A prospective clinical s...

Contemporary Reviews in Interventional Cardiology Percutaneous Coronary Intervention With Stent Implantation Versus Coronary Artery Bypass Surgery for Treatment of Left Main Coronary Artery Disease Is It Time to Change Guidelines?

On the basis of clinical trials comparing coronary-artery bypass grafting (CABG) with medical therapy, current guideline recommend CABG as the treatment of choice for patients with asymptomatic ischemia, stable angina, or unstable angina/non-ST elevation myocardial infarction who have left main coronary artery disease. Percutaneous coronary intervention can be selectively performed in patients ...

Interventional Cardiology Adherence of Catheterization Laboratory Cardiologists to American College of Cardiology/American Heart Association Guidelines for Percutaneous Coronary Interventions and Coronary Artery Bypass Graft Surgery What Happens in Actual Practice?

Background—The American College of Cardiology and the American Heart Association have issued guidelines for the use of coronary artery bypass graft surgery (CABG) and percutaneous coronary interventions (PCI) for many years, but little is known about the impact of these evidence-based guidelines on referral decisions. Methods and Results—A cardiac catheterization laboratory database used by 19 ...

The Society of Thoracic Surgeons practice guideline series: Antibiotic prophylaxis in cardiac surgery, part II: Antibiotic choice.

BACKGROUND Patients with chronic severe angina refractory to medical therapy who cannot be completely revascularized with either percutaneous catheter intervention or coronary artery bypass graft surgery present clinical challenges. Transmyocardial laser revascularization, either as sole therapy or as an adjunct to coronary artery bypass graft surgery, may be appropriate for some of these patie...
